She revealed that she has Reinke’s edema, a vocal-cord disorder, along with a large polyp that contributes to the distinctive tone and texture of her voice. Most importantly, Cyrus said her lifestyle did not cause the condition. Her voice, she explained, had always sounded this way because of her anatomy.

That revelation changed the way her career sounded in retrospect.

The same voice that once belonged to Disney’s Hannah Montana had become one of the most recognizable instruments in contemporary pop. But Cyrus has also acknowledged how difficult performing can be. She compared singing with her condition to running a marathon with ankle weights, and has said she is unwilling to risk surgery that could potentially change the voice that has become part of her identity.

Behind that professional struggle was a much larger period of upheaval.

In 2018, the Woolsey Fire destroyed the Malibu home Cyrus shared with Liam Hemsworth. The couple rescued animals as the fire approached, but much of what they owned was lost. They married later that year, only to separate in 2019 and finalize their divorce in 2020.

Around the same period, Cyrus underwent vocal-cord surgery and was forced into a period of silence. For someone who had spent most of her life performing, losing the ability to use her voice was more than a physical interruption. It became a confrontation with who she was when the stage disappeared.

Then there was her family.

Miley’s relationship with her father, Billy Ray Cyrus, has become strained, particularly following his divorce from Tish Cyrus in 2022. Tish has spoken publicly about how painful the end of their nearly 30-year marriage was, describing fear of being alone and the emotional collapse that followed the deaths of her mother and the breakdown of her marriage.

The family’s divisions became increasingly visible, but the complete story has never been as simple as choosing a villain.
